Large crystals with the shape of “shocks of wheat” in the urine of a patient under treatment with amoxycillin, 1 g every 8 hours/day (urinary pH 5.5). Even though infrared spectroscopy had not been performed to confirm the nature of the crystals, it is reasonable to hypothesize that they are made of amoxicillin due to this drug intake, crystal uncommon morphology, and acidic urinary pH.
