A large erythrocytic cast (arrows) containing a high number of erythrocytes embedded within the hyaline matrix. The presence in the same microscopic field of a high number of erythrocytes, >5% of which are acanthocytes (circles), does suggest a glomerular bleeding, as can be caused, for instance, by active proliferative glomerulonephritides.
